A monthly billing statement from each creditor. 
It must have the payment address listed on it.  If you do not have any recent statements it's ok.

Your free credit report www.annualcreditreport.com. 
If you don't have internet access or a printer we can take care of this when we meet with you.  Please don't give us any merged reports because they're garbage.

Creditor correspondence.   
Got letters from collection agencies or lawyers?  We need them so they can be properly listed in your bankruptcy so you can get your life back.

Paystubs.
We need every paystub you have been issued for the past 6 months, beginning with the month PRIOR to seeing us.  This is for the Means Test which qualifies you to file Chapter 7.  If you don't have them have payroll give you a printout that lists all paystub details (gross, deductions, and net).

All lawsuits within the last year. 
If you are suing someone or someone is suing you we need a copy of the documents so it can be properly listed in the Statement of Financial Affairs.

Annual Gross income.
We need your year to date gross income & prior 2 years.
